IMHO, no more than about 15% of protracted pauses are actually related to a difficult decision. Most of the time, it's an out of line move that gets picked off, and the mover is trying to save face...

Some people truly believe that the order of the cards once the shuffle is complete should not be disturbed (and the casinos reinforce this idea by the way some misdeals are handled)....

My basic philosophy is that defending miniscule blinds with resteal or call/donk moves either wins tiny pots or loses big ones.

Except in those cases where you're doing it as a setup to stack...
