Bodog Verification Form

Hi,
I'm trying to cashout with bodog and I have to fill out the Credit Card Verification Form. The card I used is my bank card and I believe it can be used as both credit and debit. The Verification Form asks, "Purchase Limit with Credit Card Issuer." Since it is my bank card, I don't think there really is a limit. Should I just leave that part blank? It also asks "Requested monthly purchase limit." What exactly do they mean by this?

Requested monthly purchase limit is likely a voluntary limit which you'd like at Bodog with that card to stop you from bankrupting yourself.

for the purchase limit just put n/a. that's what they told me to do. leave the other blank also.
