Anyone have any idea of the cost of replacement? Does the individual pane need to be replaced, or the entire thing?

individual pane. just call a glass place/contractor/whatever. shouldn't be too expensive. $50 maybe? depends where you are, I guess...

See if you can replace it yourself. If the glass is being held in on one side with normal glaze, you can just remove the glaze with a knife, put the new peice in there and glaze the window back in.
