Sites that require faxed materials to cash out

Stupid Golden Palace. I was scared for a minute because I have no utility bills in my name, nor even a cell phone. It looks like they'll accept a voided check, but man this sucks. Hopefully Doyle's Room is different, because I want to play some badugi.

What other sites have people had to send in documentation for? I've withdrawn from Code Poker (Prima), Poker Room, Interpoker, Stars, Party, Empire (blech!), Poker4Ever, Bodog, CD Poker, Noble, Pacific, and Poker.com, and I have NEVER had to do this before.

Goldem Palace is the only one that does it for everyone. Others may ask for it if there is a red flag, most often associated with use of a debit or credit card. If you use Neteller only GP has required this hassle.
