I think 3rd and 4th were played well, but on 5th, I think you should fold to the raise. Actually, I'm not sure if you should have bet at all.

Given that he limped with a 4 doorcard that is now completely dead, and jammed 4th after catching another heart, the only plausible hand for him to have is a flush on 5th, so I would consider check-folding since you likely have the worst hand and only 2 outs to improve.
