If the exact burn card was clearly identifiable, then it should have been taken back to be used as the proper burn for the turn, and the flop let stand. Just "that the seven was clearly part of the three card flop" is not enough. If the proper burn is unclear, then this decision is fine. I checked two rulebooks and they agreed on this. That does not preclude the possibility that the written rules might be different elsewhere.

I'm not sure what you mean here but the proper ruling is the 1st burn card stays as is......a burn card. Then the 4 cards get reshuffled back in the deck and and the dealer does not burn another card. Dealer sends out the flop followed by burn and the turn and burn and the river. I have never known any other way. OH and a quality floor might announce no rake for that hand for the confusion if a cash game.

No way. The rake is often regulated and under raking can be just as illegal as over raking (depends on jurisdiction).
