Default Taking Insurance

It seems to me that if the only consideration is EV

you always take insurance if you are behind and it is offered,
and you never take insurance if you are ahead.
But...if you are ahead it might be ok to insure against opponent with 2- 5 outs.

by the way, where can I find an insurance table )as posted in card rooms) for no limit? thanks...

If you can somehow get someone to give you insurance that's actually +EV, then sure, take it.

You will never find this. You will almost certainly have to pay a premium. For instance, if you are a 10-1 favorite, you're probably only going to get something like 8-1 from anyone willing to 'insure' you.
