Stayed at the Wynn during a week in July for $129/night with the poker rate. Absolutely worth it, especially if traveling with wife/girlfriend.

Monte Carlo is $75/night with 4 hours of play/day. Small PR though.

For a poker rate you book a room the standard way and then ask for the poker rate after you have qualified correct?

Or is it something you have to ask for up front?

You book through the poker room. Usually they book you at the regular room rate, then take the reduction when you have carded the required hours.

I'm staying at Mandalay for $90/night at the end of Aug.

I call the poker room to make my room reservations, or call the standard reservation number and ask for a poker rate.?

Call the poker room. You have to book through them usually, but even if the procedure at your hotel is different, they'll at least know who to talk to.
