How long was she looking at it before you saw it? What was her reaction before you saw it?
You may ([censored], definitely) need to do some sleuthing- if that really is the picture from the phone, you might be able to match the dude's leg with one of her 'friends.' You need to check out her phone when she's not around, and check her credit card bills.

If my wife of 5 years is getting sent text messages and pictures of slongs from dudes, I'm gonna get to the bottom of what's going on, come hell or high water.

[/ QUOTE ] For some reason I think there might be gradations between that and her receiving from what I can tell is a totally unsolicited, misnamed text message from someone not in her phonebook. Plus my read of her reaction says no cheat.
