Default Raise rules question with players all-in

Hi,

I have questions about the rules when a player goes all-in.

Player A bets $10, Player B calls and Player C raises all-in to $14. Is Player A allowed to raise now or can he only raise if Player C had raised $16 all-in (more than half the previous raise)?

Is there a difference in fixed limit and no limit? Is this rule a general poker rule or is it site dependent?

Another question:

In a $10/$20 fixed limit game, Player A bets $1 all-in. Can player B now raise to $21 or can he only complete (raise to $20)?

Thanks for your help,
Michael

One more example:

In a game with $10 big blind, the player posting the big blind posts $9 all-in (I believe in a tournament that is possible). The next player has to call $9 or $10?

Is there a difference in fixed limit or no limit? Is there a difference if he posts $9 or $1 all-in?

The raise has to be greater then half the bet to be considered a raise. Example 1 player C needed to raise to 15 or more for player A to reraise.

Example 2, he can only complete to 20

example 3 the players must follow table stakes so they will all have to call 10.

Its a general rule and theres no differance what he posts, players need to follow table stakes.

Flop/pre-Flop or Turn/River? If Player B has not acted yet, he is allowed to bet.

Example 3: Pre-flop, you must call the minimum bet, make a raise of the minimum bet, or fold.
