-6/-5. Does anyone know how to convert that to 20/x format?

Diopters don't easily convert to Snellen chart values. Snellen charts don't really offer much value informationwise, as they are strictly comparison based on an arbitrary design. The diopter measurements give you an actual scientific (non-SI) data set.

Snellen charts & Diopters
Snellen Charts from wikipedia

Someone did write up a correlation chart but I have no idea how accurate it is.

Here's a clip of part of it:

A. -0.75 .... 20/30
B. -1.00 .... 20/40
A. -1.25 .... 20/50
B. -1.50 .... 20/60
A. -1.75 .... 20/70
B. -2.00 .... 20/80
A. -2.25 .... 20/100
B. -2.50 .... 20/120
A. -3.00 .... 20/150
B. -3.50 .... 20/200
A. -4.00 .... 20/250
B. -4.50 .... 20/300
A. -5.25 .... 20/400
B. -6.00 .... 20/500
A. -6.75 .... 20/600
B. -7.25 .... 20/700
A. -8.00 .... 20/800
B. -9.00 .... 20/1000
A. -10.00 .. 20/1200
B. -11.00 .. 20/1400
A. -12.00 .. 20/1600
B. -13.50 .. 20/2000
A. -15.00 .. 20/2400
B. -16.50 .. 20/2800
A. -18.00 .. 20/3200
B. -20.25 .. 20/4000

Some research that disagrees ...

In Diopters 20/something
-0.50 20/25 to 20/30
-1.00 20/30 to 20/50
-3.00 20/300
-4.00 20/400
-5.00 20/600
-6.00 20/800
