Despite the additional toughness, you have to love the 0.6 BB/100 raise you got by promoting yourself from 10-20 to 20-40 (due to the rake effect).

I just modified my post after reading Lmn's more precise out-counting for the case when villain has the bigger king.

It makes Josh's situation less grim but I agree that it's still a fold.

I think pre-flop is sketchy; I would be surprised if this was +EV.
I think flop is fine. If you're ahead they have at most 5 outs combined against you, so the free card you're giving is not...
