1) Almost always. I can make it to about 30 seconds now though, but then...*poof*
2) More than I wish. This happens at work a lot. 3) I'm not a big flipper at commercials, but not very often when I do it. |

1) All the time. Before college I never forget a name and a face. Since it's been awful.
2) Occasionally, but not too often. 3) Not much. 4) Making a note to remember songs and never doing it: All the time. It happens to me a lot when I listen to college radio in the car. I hear something new, and I'll either remember the band's name or memorize some lyrics to google when I get home. Then I walk through the door, do my normal routine and completely forget. 4 hours later I remember I wanted the song, but can't remember any of the pertinent info. |

I forget the names of people so much that I've just stopped listening when they tell me. I then realized that names are pretty meaningless so its not a big deal. I'd rather remember some good thing about the person or where I know them from than their name.

The most annoying of these sort of instances is when I call someone with a specific question or discussion point in mind, then after the requisite small talk I draw a blank. I know that I had something interesting in mind and it was the very reason I called, but the thought has completely escaped me.

Pretty much all the time on all 4 and I've always been that way.
A semi-related one is where I'm walking around looking for my car-keys or sunglasses or something and it takes me 1 or 2 minutes to realize that I'm holding them right there in my hand. Yes, I'm really that dumb. |

1-2 all the time. 2 is especially common if my wife asks me to do something. Seemingly 90% of the time, I'll forget it within a minute. When I think of it myself, it's not as common for me to forget.
3 used to happen to me a lot, but now I have a DVR, so I rarely even watch commercials. 4, I remember this one song where, everytime it came on the radio, I'd ask my buddy who sang the song. I didn't even realize I was doing this until a few months later (after asking him everytime we heard the song), he shows up with the CD in hand, complaining that I had asked him who sang that song at least 8 times. |
