Default Odd rake policies- what have you seen?

So having heard a little about Turning Stone today and the particularly odd rake policies they used to have (which are still a bit strange), I'm wondering what other kinds of rake structure are out there in the world. I've only played in AC, Vegas, and London, but I'm sure there are other structures.

The ones I know of are:
-x% to $y max (as is the standard online)
-time collection per half hour
-graded intervals- when I played 15/30 at the bellagio was something like $1 at $40, $1 at $80, $1 at $140, $1 at $200. Turning Stone apparently does somethign similar to this, though with a higher rate.
-I heard of a game once where the small blind was dropped every hand at the beginning of the hand.

How do things work in SoCal? NorCal? Cruise lines? Oklahoma? Djibouti?
