Any time you attempt to cash out more than $5k on PokerStars you are given a Bank Wire option. It's usually extremely fast and I believe there is no maximum.

My bank charges me 4% on bank wires. Is anyone in the US avoiding these bank fees?

You need a new bank. I've never been charged a fee for receiving a wire at any bank I've banked with, that's absurd.

There is usually a small ($35) fee for initiating one though but PokerStars picks up this tab.
