Caro might have been talking about limit holdem.

In NL holdem, you should be developing a relationship with BB that includes a variety of actions that all hinge on the type of player that BB is.

The biggest factor is that you are out of position. So if BB is a good position player, then I would be folding a lot. If BB was tight, I would be raising a lot. If BB was loose passive, I'd be making value plays, limping with speculative hands, raising good ones. And so on....
