Default Notes

I find online notes very helpful but are difficult to keep live. Lately I have been taking a small notebook and taking notes on hands against players in a clandestine manner as there are many regulars at the game I play. Generally speaking I take notes on just a couple of hands over a 5 hour period indicating the way a player played a key hand. I may note if he bluffed and how, what a small or big raise PF ened up meaning if shown down and what big bets or raises ended up meaning. I also like to note if I generally think the guy is tight, loose, aggressive or what. I will also note any tells I think I spot.

Anyone else take live notes? How do you keep notes (notebook, cell phone, PDA?) what do you keep track of and do you review the notes?

I don't take any notes. If the opponent isn't a regular, I don't really have a decent sample size. If the opponent is a regular, I don't need them. I know how he plays.
