Just refuse the appetizer if it means that much and insist the restaurant mail it to Africa. That's a win win win solution if I ever saw one.

I'd be lying if I said this didn't annoy me a little, but it definitely doesn't come close to preventing me from enjoying the meal or even bothering me to the point where I'd complain.

Stuff like this happens so frequently these days at lesser restaurants that I practically expect it now anyway.
If they get it right then they get a decent tip.

If it's screwed up somehow or comes out cold or whatever then that might come out of the tip but if they are still friendly enough or I see they have a zillion tables they are working really hard to try and cover I will usually tip them okay anyway.

At a nice restaurant where I'm paying a decent amount for the food and thus the tip as well I expect that they should be able to get it correct and this almost always is the case.
No nice restaurant I can think of would actually do something like this and not be super-apologetic for it without you having to bring it up.

If you no longer want them, send them back.

You have to be 100% sure you were 100% clear with the waiter about your desire to have the apps delivered before the other food.

i would bug the waiter long before 20 min passed for a salad.

2 and 3 seem fairly normal too me.

I have never complained about a meal I have paid for unless it was inedible. If you absolutely can not eat it for any reason then there is reason to complain, if not, eat it an quit bitching.

examples, you are jewish and order a filet mignon. upon recieving the steak you realize they wrapped it in bacon, complain and send it back cause you don't eat pork, wtf!

you recieve an undercooked/overcooked meal that is unsafe to eat

you recieve a meal that has a foreign object in it that is not meant to be eaten

you get a course that is so obviously out of place that you have to send it back. there are some arguments itt that count, 2 hot courses at once count. otherwise, figure it out and eat your food.
