Default Any good books on playing small ball?

Title says it all. I hear the term, I get the general concept, but I would love to read a book that has all explains all of the nuances of playing small ball. So any recomendations?

I'm fairly sure the best material available is Erik Lindgren's book and Gavin Smith's chapter in the Full Tilt Tournament Stratigy Guide.

Negreanu plans to cover this extensively in his book. If, of course, it ever actually gets done and out to the public.

I have the understanding that HOH is the small ball bible. I listened to a show on Holdem Radio with Blair Rodman talking about Kill Phil and he characterized HOH as small ball strategy.

HOH would seem to preach much to tight a style for small ball.
