The piano coda was written by Jim Gordon, the Dominoes drummer (maybe the most underrated drummer of the era, the guy was amazing). Gordon later went nuts, killed his mother, and was sent to an insane asylum (still there I think).

When Clapton released the acoustic version of Layla from his Unplugged show he had to share the writing credit with Gordon, even tho the outro does not appear on that version of the song at all.

The slide guitar part of that by Duane is considered one of the best slide solos of all time. Most of it is played above the fretboard, so he is hitting most of the notes 'by ear'.
