Is my initial bet wrong? Fold instead of 1st call? Fold instead of 2nd call?

I would have raised all in on the flop. You have a very strong 2-way hand and you would like to get rid of better flush draws. I also want the straights to get their money in now, because they may not get it in if a heart hits. That's the only thing I would have done differently.

Nut low draw, outside straight and a weak flush draw? That can't possibly be a fold in multiy way pot.
