OK this one was seven years ago but it was so bad I have to mention it whenever discussions like this come up. Dusty Baker should have been fired in the middle of the game.

Not a single instance, but honorable mention to Joe Maddon for more than doubling up the next highest team in CS with a team that is hitting HRs at breakneck pace.
