Four times. One was for racialist remarks directed at a, well the preferred nomenclature is Asian American. Two were for telling this dude to seriously consider killing himself (I wasn't being serious though). The last one was for telling the same guy "I know where you live ha-ha" and "Remember to lock your doors he-he."

haven't been banned here yet. if any of you read TotalFark, I got banned there a few times for "parody threads"... basically some idiot submitted a series of 4 or 5 threads asking for information on how to download, install, configure firefox. I submitted a thread a headline of approximately

"HALP HOW DO I USE MY BROWZAR?!??!! TEH INTARNET IS HARD!!1!!1!!"

then once again for "abusing moderators" ... thread submitted was something like "STOP DELETING MY THREADS YOU BASTARDS"
