Theory and practice pages

I bought the Sklansky/Miller book. It's unread, as far as I can tell. But after page 4 comes page 37. Then much later in the book I find pages 22-36. Seems I'm missing pages 5 through 21.

Is this a common printing error? If so, are the missing pages available somewhere?
