I have a funny feeling that Red Sox losses will dominate this thread. I'll cast the first vote for Grady.

It hindsight (Unless you happen to be Tim McCarver, who said it just before it happened), but Luis Gonzalez hits into an easy double play if Torre doesn't have the infield in.

IMO, Francona made an ever worse mistake than this the following year by brining in his game one WS starter when it was 8-1 in the 7th inning of the 7th game of the 2004 ALCS.

I'm sure neither of these are even close to the worst ever, though.

No, it was a pop-up. Jeter probably catches the ball if he was at normal depth, but you have to play your infield in there with the bases loaded.
