Default 200NL 500BB deep 99 underfull hand...

Villain is a pretty bad player, and has made terrible calldowns against me before. I have never seen him check/raise the river, and I have never seen him showdown a big bluff of any kind.

Sucks, but I think you have to call here. For $300 more in a $990 pot, I think he has JT or trip kings enough times here instead of KQ

i.e what sort of hands does he raise/limp with preflop

As I rule I pretty much always bet 2/3 pot on the flop whenever I bet. This one was a little bit less, and probably should have been $21 or $24, but for me this is pretty standard.
