why wouldn't you have the speaker implant in your earlobe or ear, and the microphone in your lip or something similiar. could be as jewelery (earring + lip or nose ring) or as stealth implants. hell, you could have a speaker implant in each ear, so you have automatic stereo headphones on.

How about a way to have the caller specify the urgency of the call- one ringtone for a "just calling to chat" call, and one for urgent matters.

Interesting ideas in this thread, but there's really only one I care about that hasn't been mentioned: make them waterproof. Seriously. Digital calculator watches were waterproof when I was growing up 20-25 years ago, I'm hoping there's some way that one day the same can be done for cell phones, as basically every phone I've broken has been a consequence of it getting fried after getting wet.
