That book was called "Poker at the Millennium, (Hold'em and Omaha)", and was published by Mike Caro University. There is a review of it here.

Interestingly, Cardoza does not seem to have the new title on its web site.

I see that Amazon is shipping this book now... does anyone have it yet to comment on the contents?

Avery Cardoza told me it was an highly edited re-print. The info is the same but made more readable.
