How much does a box at a casino usually cost?

It should not cost you anything. You will need to leave a deposit (depends on room for amount) when you withdraw chips (cash). When you put the chips(cash)back you sign off on it and leave.

My major concern is access. Are there accounts that will let me withdraw more than a couple hundred dollars by ATM?

Some banks allow $1000. Do some research of the banks in your area.
