Default Missing Blinds (2 dead small blinds)

I was playing at the Borgata (3/6) and the player to my right had missed his blinds and was posting. He placed a $5 dollar chip for his post and the dealer made change and put $2 in the pot and $3 in front of the player as his post.

I saw this, and asked why there was $2 in the pot and was told because he'd missed two blinds. I think he meant a small and big blind. He started dealing so I shut up and tried to remember if I had ever seen that happen before.

After the hand, I'd decided that I had never seen it before, so asked the dealer again why he'd taken two dead dollars from the player, but I didn't get any better of an explanation. The player who posted didn't seem to care so I just shut up again, but I'm curious:

Is there any time a player puts two dead small blinds in the pot and posts the big blind?

Edit: Please don't make this a dealer bashing thread. I would rate ~75% of the dealers I get at Borgata as being excellent, 20% Average/Acceptable, <5% bad. That seems pretty good to me.
