Neteller Card ATM withdraw problem!

Today I used my Neteller card at a bank ATM, and withdrew 400 USD. However, the machine never spat out my money, it gave me a receipt and my card back, but never dished out $$. The bank reps said they dont service the atm, that I had to contact my bank (Neteller) and then fill out some form so they can check with the ATM people whether it spat out the money etc, then it will be put back in my Neteller account. Has anyone ever had to deal with this hassle before, and can you give me instructions on what to do? Thanks!
