Getting to Know You... OOT Style

I think one of these threads have been done before in OOT, but I couldn't find it. I know the SS forum did one a long time ago.

Anyway, I had thought about doing this for awhile, but never got around to it. Then a week or two ago, some "introduction" threads began popping up. In one, El Diablo offered a suggestion for a general introduction thread for everyone (like this) as a solution for a possible flood of intro threads. Luckily, the individual intro threads quickly died down. However, no one ever made a mass intro one, either. I didn't think I was the best person to do this, so I was hoping someone else would start it. Obviously, that didn't happen.

So... here's the deal. It's pretty simple. Cut and paste the following in a reply and fill in the blanks:
