This is a semi-rare one, a foreign film. I highly recommend watching this to anybody, it is mind-blowing, even if I don't put it as my MOST disturbing.

Weird, this thread was inspired by 'Requiem...' and that's my pick.

'The Cell' also deserves consideration.

this movie pretty much kills every other movie in here. you have insane types of violence, incest (brother sister), forced incest (father daughter), a guy getting his teeth ripped out, a guy cutting out his own tongue.

Its a great movie but hits a lot of what Id consider "real horror" right on the head.

On a more mainstream level, i.e., a movie that more than a handful of people might actually have seen, I'd nominate The House of Sand and Fog.
