Default Cash out on bookmaker?

Is this normal for a regular check by mail (free)? I tried to chash out $100 and I received this email in return.

email a copy of the credit card(s) you used (front and back), a picture ID
and an authorization statement which is attached to this message; please

Also on the to line it was to someelse. fkbaker1001@verizon.net, which is not my email.


Attached to it is 2 forms asking for card number and information. The security number on the back. Purchasing limit, name of bank. And it wants a lot of other stuff. Is this normal or should i call bookmaker?
