IME most girls say it meaning "made out" and most guys say it meaning "had sex" or they want to intentionally be vague about it so that they look cooler by making people think they had sex.

yeah, basically, 'hooking up' is made to sound like you did more then you actually did. It SHOULD mean sex, but is left to the listener to acctually decipher what it means.
