My ruling would be that in the first case, sucks to be you, but you have to make it to the add-on period to add-on.
In the second case, you can re-buy, but it only gets you up to T4000. The other 20 chips are lost. It wouldn't be fair for them to go to a specific person, and at a MTT, I don't think it'd be fair to go to a specific table.
