It was basically handled in the standard way, with two exceptions:

1. Even though the dealer knew the proper resolution, he should have called the floor over, and let the floor tell him what to do. Doing this shows the players that the dealer isn't making up some procedure to try to get out of the jam that has been created.

2. Pulling back the three was correct--but immediately putting cards back into the deck is not the SOP. The three should be set aside while "the card that would have been the river card" is put out. THEN the three is shuffled into the remaining stub.

Again, the procedure was, for all intents and purposes, followed appropriately, with a few hiccups that affected the perception of propriety.
