Bring the tailor something to copy, pictures at least. In Bangkok there are several excellent (usually Indian) tailors that can make brilliant knockoffs in your choice of color/material. They're less skilled at designing Western clothes out of whole cloth.
Also, custom silk shirts are sick cheap in Thailand.
