After looking at this image more, I have come to the following conclusion as to what is really happening:
1- Aids girl and guy next to her are having some fingering action fun. (Look at his expression/where he is looking, and hers)
2- Guy to their left is enjoying the show he is getting, under the table. (Notice the right hand, and his face says everything)
3- Shemale to his left: "Hay guys!!!!!! hihihi *wink wink*"
