What you talk about is something that I'm finally starting to understand and it is helping me improve my play, my reads, and especially my results immensely. I started to notice that after lots of hands I would think to myself, "wow that was stupid it was so obvious he had that, I should have folded," or, "damn, I knew this foolio had a huge range and hates to fold, I should have put in a small value bet there instead of checking so fast." The importance of doing this type of thinking DURING the hand and not AFTER cannot be emphasized enough, because when you are actively thinking during the hand you start to notice the differences between those two AQ hands and it starts to become second nature, allowing you to focus your thoughts on gaining other edges.
