Default 1000NL, OOP in reraised pot against two TAGs

I really hate the way I played this hand, but I want to know ALL the mistakes I made here.

Villain are both party regs: 16.5/13/2.8 and 17/11/2.2

CO ($1000)
Button ($1023)
Hero ($990)
BB ($2737.50)
UTG ($1314)
MP ($1445.50)

Preflop: Hero is SB with Q[img]/images/graemlins/heart.gif[/img], Q[img]/images/graemlins/spade.gif[/img].
<font color="#666666">1 fold</font>, <font color="#CC3333">MP raises to $35</font>, <font color="#CC3333">CO raises to $125</font>, <font color="#666666">1 fold</font>, Hero calls $120, <font color="#666666">1 fold</font>, MP calls $90.

Hero checks, MP checks, CO checks.

Turn: ($385) 5[img]/images/graemlins/club.gif[/img] <font color="#0000FF">(3 players)</font>
<font color="#CC3333">Hero bets $260</font>, MP calls $260, CO folds.

River: ($905) T[img]/images/graemlins/heart.gif[/img] <font color="#0000FF">(2 players)</font>
<font color="#CC3333">Hero bets $360</font>, MP calls $1060.50 (All-In), Hero calls $245 (All-In).

Final Pot: $2570.50

1) preflop. I very rarely 4bet with QQ + ranges of these two guys are pretty tight. However I dont know how I may fold QQ on a non-K or non-A flop. So, should I definately 4bet pf?
2) flop. here I think I missed a bet. Would you shove if this bet get raised?
3) river. In this situation I almost always hear: "Shove yourself". I decided not shoving on the river, cause this villain may fold medium pairs to a shove, but may call a smaller bet with 88 or 66. The problem with this strategy is that I think I should fold to a raise if I dont shove here. So, will you call as played?
