Q1: I've seen more than one live game with AA vs KK vs QQ preflop. I've seen set over set often. I haven't seen 3 sets yet on the same flop yet, but just because it's rare doesn't mean that it won't happen to you in a fair game. It's just a matter of time. Even Doyle Brunson in SSI wrote about a hand like J33 where the players had JJ vs KQs. The turn was a 3 and the river a 3, and the KQ guy won a huge pot with quad 3s, K-kicker.

Q2: I've seen set over set where the lower set made quads on the turn. I know about that quads hand because I won over $1K with it when my set of 2s turned into quads on the turn vs another guy's flopped set of 4s at a 5/5 NL game.

I'm sure the odds are low, but just because it is rare, doesn't mean you aren't going to see it.
