Check out Adams' address position here. I feel that's easier than using words. Notice the different head position. Your head is almost leaning on your right shoulder, his is being pulled up away from his shoulders from the crown of the head. I would also say your hands are fractionally too far forward which exacerbates the already poor angle in your spine.
You have a very nice looking swing considering this starting position. The change in Address position would result in much improved shot dispersion without changing your swing at all. Having said that, changing head positions is by no means an easy thing to do. It will take a fair bit of work to get comfortable.
My suggestion for the fix is to buy a small mirror which you can put outside the ball. This way you can check your angles while you address the ball and as you practice.
