First guy calls the floor and asks if his hand is dead.
Floor gives the guy a warning. Next time his hand is dead.
'All in' boy had the nuts and is lucky his hand wasn't killed.
Next time it happened it would be "You, sir, are all in and your hand is also dead."
The thing about these rules is, it states that you get one warning. After that, you're fair game.

Both these situations are way different from the examples you gave before. There's a huge difference between swearing at someone and declaring "all-in" out of turn, and asking your opponent a question in order to get a reaction.
