This really sucks, because it almost certainly means I'm going to have to move to someplace with a tech sector, and that means leaving behind beautiful Hawaii, all of my wife's family, our support network, etc. Right now we live rent free with my in-laws, which is particularly awesome due to built-in baby-sitting and all the help they give. Our whole lives are going to change because I've completely lost both the ability to play good and to run good.

wow, that really sux. but if u won over a decent sample before, Id say give urself another try and really work hard on ur game. u probably did all kind of stuff, so I guess my advice isnt really helpful, but:
take a few days off
watch a bunch of vids
play less tables!!!
find 1-2 guys to analyze each others sessions together
get a coach and sweat sessions
move down a limit for a certain amount of hands
...
look back at all the way u have come: u can do it!
maybe get a "small" job at hawaii to bring in some money while u regain confidence? the world may look different in a month.
