I take the evaluations seriously. Some questions that come to mind are along the lines of:

Was the workload for the course reasonable for the credits earned? (this question should probably have a scale that aims more towards "too much" or "too little")

Were the expectations of the class clearly outlined by the syllabus? (it is really annoying when profs don't follow the syllabus)

Would you take a class with this prof again?
