Adjustments can mean payment and write off so unless you show the real EOB from the ins co, this is just a statment from the doctor saying you owe $15. My guess is the person who data entered the payment from the insurance company posted it wrong and this is why nothing is showing up in the Insurance Payment column.
There is no way an insurance company would tell a doctor to write off ALL of the claim except the patient's copay. Just wouldn't happen.
