I have problems like this that creep in (mostly with my wedges).

The first thing I do is get out a roll of masking tape, tape the club face.

Now hit 2-3 balls and see where I'm hitting it. I swear there are times I think it's the toe and it's actually the hosel.

I've found that sometimes it is how close I'm standing to the ball but also how steep the angle of my swing is.

Going through this process has actually been my greatest increase in ball striking ability ever. It's great feedback to see exactly where you are hitting the ball.
