For the most part, if I sit at a table with 2 players, I will sit out if both players say they want HU, NOT just one, as that's usually the shark trying to carve up a fish. There have also been instances in the past where I have played even though they both said HU please because one or both of them were fish, and while being nice is nice, I'm not playing poker to be nice.

Plenty have/can do it in real life, although a decidedly smaller amount of people pay HU $$$ live in casinos. A famous example is Ted Forrest walking in on an easrly incarnation of the big game with Beal and making it 3-handed for a time.
