Being drunk would be bad anyways as it thins the blood and dilutes the color of the ink.
It really depends on where you're getting it. Some places hurt like hell. But assuming your first will be on your back or arm or something like that, the pain is not too bad. I'd consider it more annoying that painful. It feels like someone is pinching you over and over again. But after a few minutes you get used to it.
